GLP-1 nausea: how to still get your protein in
GLP-1 nausea is the most common side effect, reported by 25% to 44% of adults in weight-loss trials, mostly during dose increases (FDA labels). If you can keep food down, eat 10 to 17 g of protein from cold, low-fat foods every 3 to 4 hours. If you cannot keep fluids down, call your prescriber.

How common is nausea on a GLP-1, and when does it happen?
Nausea is the most common side effect of GLP-1 medicines, reported by 25% to 44% of adults in the weight-loss trials behind the Zepbound and Wegovy labels.
The 2025 joint advisory from four US nutrition and obesity societies gives the same range and says GLP-1s delay gastric emptying, which leads to bloating, fullness and nausea. The Zepbound and Mounjaro labels say most nausea, vomiting and diarrhoea happened during dose escalation and decreased over time. Nausea eases with time on a given dose.
The advisory adds a trap that matters for muscle. It describes people who get caught in a cycle of not eating because of nausea, which worsens the nausea and makes eating less likely. Reduced appetite and food aversions make adequate protein hard to reach. Small cold portions break that cycle, and the menus below show how.
What do the labels say about nausea rates by drug and dose?
Label nausea rates range from 11% to 44% on active drug, against 4% to 16% on placebo, depending on the product, dose and group studied.
The table lists nausea rates from selected adult trials in the current US labels. The label tables report dose arms of each trial, not escalation weeks, so they show how nausea differs by dose reached and not by how long you have been on it.
| Product (population) | Dose arm | Nausea on drug | Nausea on placebo |
|---|---|---|---|
| Wegovy injection (obesity) | 2.4 mg weekly | 44% | 16% |
| Wegovy injection, pooled higher-dose trials (obesity) | 7.2 mg weekly | 39% | 13% |
| Wegovy injection, same pooled trials | 2.4 mg weekly | 35% | 13% |
| Zepbound (obesity) | 5 mg weekly | 25% | 8% |
| Zepbound (obesity) | 10 mg weekly | 29% | 8% |
| Zepbound (obesity) | 15 mg weekly | 28% | 8% |
| Ozempic (type 2 diabetes) | 0.5 mg weekly | 15.8% | 6.1% |
| Ozempic (type 2 diabetes) | 1 mg weekly | 20.3% | 6.1% |
| Mounjaro (type 2 diabetes) | 5 mg weekly | 12% | 4% |
| Mounjaro (type 2 diabetes) | 10 mg weekly | 15% | 4% |
| Mounjaro (type 2 diabetes) | 15 mg weekly | 18% | 4% |
| Rybelsus (type 2 diabetes) | 7 mg daily | 11% | 6% |
| Rybelsus (type 2 diabetes) | 14 mg daily | 20% | 6% |
Three points help you read it. Rates inside one product rise with dose in most rows, but not every row: Zepbound 15 mg (28%) is slightly below 10 mg (29%). Compare doses within a product, not across products: trials differ in people, length and design, as the labels say. The pooled Wegovy 7.2 mg trials show how much the comparison group matters: 2.4 mg gave 44% in the original trials and 35% in the later ones.
Nausea is expected enough that the labels build dose steps around it. The Wegovy injection label starts at 0.25 mg and steps up every four weeks, and it says to consider delaying escalation for 4 weeks if a dose is not tolerated. Zepbound starts at 2.5 mg and rises in 2.5 mg steps after at least 4 weeks. If a step hits hard, ask your prescriber to delay it.
Does the Wegovy pill cause nausea?
Yes. In the OASIS 4 trial, 46.6% of people on the 25 mg Wegovy pill reported nausea (95 of 204), against 18.6% on placebo (19 of 102).
The nausea figures come from the posted trial results. The trial ran 71 weeks in 307 adults and was published in the New England Journal of Medicine in 2025, where 74.0% on the pill had some stomach or bowel side effect, against 42.2% on placebo. The older Rybelsus label for type 2 diabetes lists nausea at 11% on 7 mg and 20% on 14 mg, against 6% on placebo.
The pill adds a timing rule that interacts with eating. The Wegovy label says to wait at least 30 minutes after taking the tablet before eating, drinking or taking other medicines, and to swallow it with up to 4 ounces of plain water. If nausea hits in the morning, start your first small protein serving once the 30 minutes pass. The Wegovy pill morning plans lay out the schedule.
What should you eat when you feel nauseous on a GLP-1?
Eat small portions of protein every 3 to 4 hours, starting with a small breakfast, and keep fluids going.
That is the advisory’s guidance. It notes nausea often comes in the morning or after long gaps without eating, that smaller, more frequent meals and avoiding fatty or high-fibre foods in the first few days can help, and that vomiting is more likely after large meals. It also lists ginger or peppermint tea among options and says to keep alcohol low, since alcohol can worsen nausea and reflux.
Generic anti-nausea lists stop there. The sections below tell you how much protein each small portion carries.
What counts as a bad-day protein target?
A bad-day target of 60 to 80 g of protein sits between the advisory’s floor and its normal range, and the floor is the number to protect.
The joint advisory says protein should not fall below 0.4 to 0.5 g/kg a day, which is 34 to 43 g for an 85 kg adult. Its range for active weight loss is 1.2 to 1.6 g/kg or a flat 80 to 120 g. The 60 to 80 g band on this page is the rough-day target: well above the floor, a little under the normal range. Our protein target guide shows how to work out the full range for your weight.
Which foods give protein in small, cold, low-fat portions?
Nonfat Greek yogurt, tuna, cottage cheese, kefir, skim milk, sliced turkey and a whey isolate drink each give 8 to 17 g of protein with 3 g of fat or less per portion.
Cold foods smell less and sit easier, so the list starts there. Low-fat foods follow because the advisory says to avoid fatty or high-fibre foods in the first few days of treatment. The grams below come from the USDA FoodData Central SR Legacy data (per 100 g), multiplied by the portion weight. Portion weights are USDA household measures where one exists (one container, one cup, 3 oz, one large egg, one slice) and are set at about one scoop for the whey. Brands differ, so a product label beats this table. More ideas are in our high-protein foods for GLP-1 list.
| Food (USDA entry) | Portion | Protein | Fat | Energy |
|---|---|---|---|---|
| Greek yogurt, plain, nonfat | 1 container, 170 g (6 oz) | 17.3 g | 0.7 g | 100 kcal |
| Tuna, light, canned in water, drained | 85 g (3 oz) | 16.5 g | 0.8 g | 73 kcal |
| Whey protein powder isolate | 28.7 g (about one scoop) mixed in water | 16.7 g | 0.3 g | 103 kcal |
| Cottage cheese, lowfat 2% | 113 g (4 oz) | 11.8 g | 2.6 g | 92 kcal |
| Turkey breast, sliced, prepackaged | 4 slices, 64 g | 9.5 g | 2.4 g | 68 kcal |
| Kefir, lowfat, plain | 1 cup, 243 g | 9.2 g | 2.5 g | 104 kcal |
| Skim milk | 1 cup, 245 g | 8.3 g | 0.2 g | 83 kcal |
| Egg, whole, hard-boiled | 1 large, 50 g | 6.3 g | 5.3 g | 78 kcal |
| Tofu, silken, firm (Mori-Nu) | 1 slice, 84 g | 5.8 g | 2.3 g | 52 kcal |
The egg is the one item with notable fat, so skip it if fat is your trigger. Real tubs of isolate differ in grams per scoop, so read yours.
What does a bad-day menu with 60 to 80 g of protein look like?
Six small servings spread over the day reach 72.6 g of protein with 9.1 g of fat, and no serving is bigger than a cup.
Each line is one portion from the table above, and the totals are the sum of the rows.
| Time | Food | Protein | Fat |
|---|---|---|---|
| Morning | Skim milk, 1 cup | 8.3 g | 0.2 g |
| Mid-morning | Cottage cheese 2%, 113 g | 11.8 g | 2.6 g |
| Midday | Tuna in water, 85 g | 16.5 g | 0.8 g |
| Mid-afternoon | Greek yogurt, nonfat, 170 g | 17.3 g | 0.7 g |
| Early evening | Sliced turkey, 4 slices | 9.5 g | 2.4 g |
| Evening | Lowfat kefir, 1 cup | 9.2 g | 2.5 g |
| Total | 72.6 g | 9.1 g |
The plan holds about 520 kcal, a floor for a hard day. If you take the Wegovy pill, start the day after the 30-minute wait. Two lighter variants use the same table:
- Four-feed version, 62.3 g. Greek yogurt (17.3 g), one whey isolate drink (16.7 g), cottage cheese (11.8 g) and tuna (16.5 g). Fat is 4.4 g. Use it when four stops are all you can manage.
- Liquids only, 60.1 g. Two whey isolate drinks (33.4 g), one cup of skim milk (8.3 g) and two cups of kefir (18.4 g). Fat is 5.8 g. Use it when solid food turns your stomach.
All three sit above the advisory’s floor for an 85 kg adult (34 to 43 g). None meets the 80 g bottom of the normal range, so return to your full target as soon as nausea lets you. If you cannot eat enough for several days in a row, call your prescriber.
Is a protein shake good for GLP-1 nausea?
A protein shake delivers 17 g of protein in a small liquid volume, which makes it the easiest bad-day option for many people.
Thin, cold, low-fat drinks such as a whey isolate in water go down easier than a thick milkshake-style product. The advisory says vomiting is more likely after large meals, so sip a small serving slowly. Check the label for protein and fat per serving. Our protein shake guide compares options, and the protein target guide shows how many shakes you need.
Does anti-nausea medicine help?
The 2025 advisory says anti-nausea medicines can provide relief during dose increases, and it names prochlorperazine as possibly preferable to agents that target serotonin receptors.
If nausea keeps you from eating enough, ask your prescriber about an anti-nausea medicine, a slower dose step or a different dose.
When is nausea a reason to call your prescriber or get urgent care?
Call your prescriber if nausea, vomiting or diarrhoea is severe or lasts, and seek urgent help for severe abdominal pain, which can signal pancreatitis or gallbladder disease.
The labels give these instructions:
- Severe or persistent stomach symptoms. The Zepbound label tells patients to contact their healthcare provider if they have severe or persistent gastrointestinal symptoms. It is not recommended in people with severe gastroparesis.
- Dehydration and kidney injury. The Zepbound and Wegovy labels warn that vomiting and diarrhoea can cause acute kidney injury through fluid loss. Patients should report persistent nausea, vomiting or diarrhoea. The advisory adds that dehydration can also cause heart palpitations.
- Pancreatitis. Severe abdominal pain that may spread to the back, with or without nausea or vomiting. The label says to stop the medicine promptly and contact a provider if it is suspected.
- Gallbladder disease. The labels say patients should contact a provider for gallbladder follow-up if it is suspected.
- Serious allergic reaction. The labels list anaphylaxis and angioedema and say to seek medical attention promptly.
If you cannot keep fluids down, call the same day.
Frequently asked questions
Does GLP-1 nausea go away?
The Zepbound and Mounjaro labels say most nausea, vomiting and diarrhoea happened during dose escalation and decreased over time. They give no average duration. If nausea lasts for days or keeps you from eating, tell your prescriber, who can slow the dose step or add an anti-nausea medicine.
Can I drink a protein shake when I feel nauseous on a GLP-1?
Often yes. A small, cold, thin shake is the easiest way to get 17 g of protein into a queasy stomach, and a whey isolate in water has under 1 g of fat. Sip a half serving first. Thick, sweet shakes are the ones people most often push away.
What foods make GLP-1 nausea worse?
The 2025 joint advisory says to avoid fatty or high-fibre foods in the first few days of treatment and that vomiting is more likely after large meals. It also says alcohol can worsen nausea. Keep portions small and fat low.
Does the Wegovy pill cause nausea?
Yes. In the OASIS 4 trial, 46.6% of people taking the 25 mg pill reported nausea, against 18.6% on placebo, and 74.0% had some stomach or bowel side effect, against 42.2%. Take the tablet at least 30 minutes before food.
How little protein is too little on a bad day?
The 2025 advisory says not to go below 0.4 to 0.5 g/kg a day, because lower intakes can lead to muscle loss. That is 34 to 43 g for an 85 kg person. If you stay under it for several days, call your prescriber or dietitian.
